I give the Filotimo Foundation permission to use my name, photos, video, voice, quotes, and story (my “story”) in its materials. This includes the website, social media, newsletters, fundraising, events, presentations, and press.
I understand:
• This is voluntary. Saying no will not affect my eligibility for any Filotimo program or support.
• I won't be paid. I am sharing my story to help raise awareness and support for adults with CF.
• Filotimo may edit. My story may be shortened or edited for length and clarity, but it will not be changed in a way that misrepresents me.
• Health details are my choice. Filotimo will only share the health information I approve below.
• I can change my mind. I can withdraw permission at any time by contacting info@filotimofoundation.org. Filotimo will stop using my story in new materials, but can't recall items already printed or published.
